(2)  Annabelle Breuer-Udo, owner of Evocative Health Path LLC is an Ayurvedic Health Counselor (AHC) and Practitioner certified through the Minnesota Institute of Ayurveda. Annabelle is a certified Health Coach trained by Dr.Sears Wellness Institute, Osteopathic and Naturopathic practitioner trained and licensed in Germany, TriYoga 200-hour Basics teacher certified by TriYoga International, Mindfulness and Meditation teacher certified by McLean Meditation Institute and MSc.Clinical Mental Health Counselor in training through Capella University. 


"THE STATE OF MINNESOTA HAS NOT ADOPTED ANY EDUCATIONAL AND TRAINING STANDARDS FOR UNLICENSED COMPLEMENTARY AND ALTERNATIVE HEALTH CARE PRACTITIONERS. THIS STATEMENT OF CREDENTIALS IS FOR INFORMATION PURPOSES ONLY. Under Minnesota law, an unlicensed complementary and alternative health care practitioner may not provide a medical diagnosis or recommend discontinuance of medically prescribed treatments. If a client desires a diagnosis from a licensed physician, chiropractor, or acupuncture practitioner, or services from a physician, chiropractor, nurse, osteopathic physician, physical therapist, dietitian, nutritionist, acupuncture practitioner, athletic trainer, or any other type of health care provider, the client may seek such services at any time."

(2) Fees, Payment, Insurance: At Evocative Health Path LLC, payment is accepted in the form of card, paypal, Zelle, Venmo, and check paid directly to Annabelle Breuer-Udo or Evocative Health Path LLC. Payment for services offered are nonrefundable. Currently at Evocative Health Path LLC, no insurance is accepted with payment for services. Services available:

A. Integrative/Ayurveda Health and Wellbeing Coaching- Intake Session: $299 for one Initial Assessment - Comprehensive Intake Session up to 120 minutes plus pre/post session prep.time and recommendation e-mail.

B. Follow-up Session Integrative/Ayurveda Health and Wellbeing Coaching: $135 for one

follow-up session up to 45 minutes plus pre/post session prep. time and recommendation e-mail.

C. 3 Month Transformation Package: $499

3 Sessions Package - Integrative/Ayurveda Health and Wellbeing Coaching (includes one comprehensive intake session and 2 follow-ups plus pre/post session prep.time and e-mail support between sessions).

D.Becoming The New You: $2400

1:1 Integrative/Ayurveda Health Coaching six months signature program. Includs 12 Individual Coaching sessions, limitless e-mail support and text message support via Signal or any other agreed channel. Pre/Post session preparation time and recommendation list.


(3) Change of Price: Client has a right to reasonable notice of changes in services or charges. Client will be notified no later than 14 days prior to price change for services offered.

(4) Theory of Treatment: The Ayurvedic Health Counselor and Integrative Health Coach will perform practices within their scope of practice including, but not limited to, diet, lifestyle, yoga, exercise, herbal, and general treatment advice and care dependent upon client needs.

(5) Right to Current Information: The client has a right to complete and current information concerning the practitioner's assessment and recommended service that is to be provided, including the expected duration of the service to be provided.

(6) Personal Interaction: Both clients and practitioners may expect courteous treatment and to be free from verbal, physical, or sexual abuse.

(7) Personal Information Confidentiality: Client records and transactions with the practitioner are confidential unless release of these records is authorized in writing by the client or otherwise provided by law.

(8) Right to Self-Access: Client has a right to be allowed access to records and written information from records in accordance with sections 144.291 to 144.298;

(9) Right to Alternative Treatment: Client can seek other services that may be available in the community. These services can be located by asking the practitioner, the provider the client was referred by, or they can look up Ayurvedic health care professionals in the area.

(10) Right of Transfer: The client has the right to choose freely among available practitioners and to change practitioners after services have begun, within the limits of health insurance, medical assistance, or other health programs. Payment for services through Evocative Health Path are nonrefundable.

(11) Records Transfer: The client has a right to coordinated transfer when there will be a change in the provider of services.

(12) Right to Refuse Service: The client may refuse services or treatment, unless otherwise provided by law, and the client may assert the client's rights without retaliation by the practitioner.

Acknowledgment by client:

 Prior to the provision of any service, a complementary and alternative health care client must sign a written statement attesting that the client has received the complementary and alternative health care client bill of rights.
